I have the same problem
renstimpy11 since the latest Meta update (.77 on 17 june) and DeoVR update 14.9 i have the same problem in almost every video now. Before i only seen it happen once.

renstimpy11 META has just released version V78. It might be worth checking out to see if it addresses the issue you encountered. So far, feedback from Reddit users has been quite positive on addressing those V77 tragedies.
I am currently trying to recreate the issue that you are facing, do you mind sharing with me some of the video titles that you had viewed before that caused the refresh switching issues? You had mentioned Quest3, did you use it as a standalone or PCVR?

Try " Quest Game Tuner", I think you will be impressed. It safely overclocks the CPU and other tweaks and I'm pretty sure it fixes the issues with the Q3.

renstimpy11 If anything, overclocking a CPU that is overheating is going to make the problem worse.
@renstimpy11 , @phyuggene might be right on this though, overclocking might make it worse, as the real root cause of this is definitely the current messed-up state of the Quest Thermal Throttling mechanism.
I had taken the liberty to dig up the developer document on their dynamic throttling and run some tests by artificially inducing the thermal trigger state, and sure enough, the familiar dreaded 72hz drop rears in. If you want to check this out and are familiar with adb console command. Just run this while playing a video on any VR app to force throttling for 10 seconds:
adb shell am broadcast -a com.oculus.vrruntimeservice.COMPOSITOR_SIMULATE_THERMAL --es subsystem refresh --ei seconds_throttled 10
Seems that if the dynamic throttle kicks in, the first automatic course of action for the HMD is to force the refresh rate to native 72hz at all times. I hope that Meta will fix this up soon. Currently, all Quest video players are facing the same issue
renstimpy11 Yeah bro, i dropped my remote and noticed tilting up/down sometimes makes the video skip. Doesn’t happen a lot, but I can trigger it if I move the stick enough. Recalibration helped a bit, but I’ll probably return it, a got a year left on the warranty.
Thinking of waiting tho, maybe a new quest is coming. Any news? Haven’t kept up with VR tech lately. Vr Games are fun but don’t really keep me hooked.
renstimpy11 If you encounter "insufficient memory to run...." on the Quest, clear the cache. I believe there's an option to do so in the DEOVR app and on the quest itself. If you change settings on videos, like framing Flat videos, or adjusting color settings, those get saved in cache and can eventually fill it up. That, and a restart can usually fix the same issue.
"Quest Game Tuner" has fixed this issue for me on the Q3 for DeoVR and Heresphere.
It does cost $10 but well worth it.
I don't know the details on how it fixes the 72Hz issue and don't really care.

Thanks, installed it too and works fine. What settings for DeoVr are you using? I copied the HS settings
bazzio if you mean the settings for QGT then I believe it's just 2048×2048 120fps. Also make sure the Adaptive OC is ON in the Options (although, I haven't tested with it OFF ). Max Foveation and Adaptive Refresh Enabled, haven't really messed with those much yet.
Heresphere all the same settings too.
Also, Launching the apps from the QGT UI seems to be the way to go.
