I think the scale has to do with the IPD of the Evo he used, I used to own that camera and unless your subject is less than a foot away I do remember scale being an issue. Then again he will be switching to the canon lens and we already know how most people feel about the scaling with that system hahaha I guess nobody will be pleased with scaling unless you mod your lens like SLR has or you use a K1, K2, or FMDUO system lol

I've tried passthrough with my system as well and also defaulted to greenscreen instead of alpha PT, it was just too cumbersome of a workflow for a hobbyist like myself but perhaps he can adopt it since it seems like he may be pursuing this as a full time job eventually. I think that even if you use chromakey instead of alpha, you can simply add a quick frame with a position diagram in between each change to let the viewer know what position to change to next? I know its not ideal or some may say it breaks the immersion but hell, switching positions is already jarring as it is, so a smooth transition with a position graphic may not be that bad. I see these quite often at the start of CGI VR scenes.
The anticipation part is an interesting point, I never really put any thought into it but it does make sense, I think for the most part we are all fairly conditioned to the same type of progression or peek at the progress bar periodically to know when the climax is going to happen. But now that I put a little more thought into it I do notice that a big part of it falls on the girl performers responsibility to sort of queue it up by what they say leading to it and also the male will often start to squeeze the girls arm or leg to let them know he is close to blowing up which prompts the girl to quickly change position to finish him off. Very interesting insight.
